2026 1095-C Form Request

The District will provide 2026 1095-C Forms upon request. Requests can be sent by email or mail on or after January 31, 2026, to the address or email address listed below.

A copy of the requested form will be sent within 30 days of the request. Please include your full name, your employee id number, your address, your email address, and how you would like the form sent to you (mailed or emailed).
