Back-to-School Immunizations

Back to School Immunization Requirements

Please be aware that the Pennsylvania Department of Health amended its regulations to school immunizations, 28 Pa. Code Chapter 23, Subchapter C. These regulations were effective August 1, 2017, and will impact all students entering school year 2018-19.  

For entry into 7th grade, the Meningococcal conjugate vaccine (MCV) and Tetanus, diphtheria toxoid, and acellular pertussis vaccine (Tdap) are required.   If your child does not have proof of these immunizations on the first day of school, he/she will be sent home on the 5th day and excluded until completed.  If your child is excluded, the absence is unexcused and considered truant. Ask your childัs doctor to complete the information below and return or fax the form prior to the first day of school.
