UDHS Sports Physical Night

For athletes entering 9th, 10th, 11th, and 12th grades, we are offering sports physicals in the high school gym on Monday, June 2, 2025 from 3:00 PM - 5:00 PM! The cost is $30 per athlete (cash only, please). Athletes must be accompanied by a parent/guardian.

Please sign up here to reserve a time. If there are multiple athletes in your family, please fill out the form once per athlete.

You should complete the rest of FamilyID/Arbiter Registration for Fall 25-26 before your appointment. Since we need to provide a copy of the athlete's health history (CIPPE Section 5) to the doctors, please print out that section from FamilyID/Arbiter Registration or be prepared to fill it out when you check in for your appointment. Register for Fall 25-26 on FamilyID/Arbiter Registration here.

When should I register for a sports season?

As soon as possible! The earlier you register, the quicker we can review the registration and help you fix any errors or omissions. Your registration will not be approved until it is filled out correctly and completely. 

How do I register?

If you are planning to join a Fall sport, you must sign up and be approved on FamilyID/Arbiter Registration BEFORE tryouts begin! Please follow these steps:

1. Go to FamilyID/Arbiter Registration by clicking on this link.
2. Select the sport(s) you wish to try out for and fill out the registration form.
3. Upload a photo of your PIAA sports physical in the Participant Information section. Make sure it is dated after May 1, 2025 and that a full, non-blurry photo is clearly visible on the screen.
4. Double-check your signatures in the Agreements section! Make sure a parent/guardian signs the fields marked 'Parent/Guardian' and a student signs the fields marked 'Student'.
5. When you have finished filling out the form, click 'Continue'.
6. On the next page, click 'Submit'.
7. Check the status of your registration by signing into FamilyID/Arbiter Registration and clicking on the Registrations tab in the menu. Your registration must be marked 'Approved' in order to try out! If there is a problem with your registration, the status will be marked with 'Corrections Required' and an email will be sent to the account owner with a description of what needs to be fixed. You can also ask Ms. Kramer in the Athletic Department office to explain any issues. 

Note: If you do not complete this process, you will not be allowed to try out for any sport. You must check in with the Athletic Office to make sure your registration has been approved.
