    Sports Physicals and FamilyID FAQ

     

    Sports Physicals:

    What is a sports physical?

    A sports physical is the comprehensive physical evaluation given by an authorized medical examiner that ensures the student athlete is medically cleared to play sports. We use Section 6 of the CIPPE form, which is mandated by the PIAA.

     

    How do I turn in the sports physical?

    You will attach a digital copy (either a photo or a scan) of your sports physical to your FamilyID registration. For more information on FamilyID, see below.

     

    Can I just turn in a paper copy?

    No. By using FamilyID, our athletic trainer, coaches, and administrators will have instant access to the sports physical in case it is needed, while you get to keep a hard copy of it for your records.

     

    Where can I get a sports physical?

    Take a copy of Section 6 of the CIPPE (available outside the Athletic Department in hard copy or available for download here) to your doctor after June 1.

     

    What if I already got a physical, but it's not a sports physical?

    We will accept any official medical form as long as it contains the same information as the CIPPE Sports Physical, the medical examiner clears the student athlete to play sports, and it is dated after June 1, 2019. If the form does not have this information, we cannot accept it. Please take Section 6 of the CIPPE back to your doctor and have them fill it out.

     

    When do I need to get a sports physical?

    You need to get a sports physical after June 1 (so June 1, 2019 for the 2019-2020 school year). June 1 was chosen by the PIAA to ensure that all student athletes have an up-to-date record of their health.

     

    What do I do if the sports physical is dated before June 1?

    We know that some insurance policies only cover one physical a year. If that took place before June 1, please contact your doctor so they can fill out a current form (dated after June 1, 2019) for your student athlete.

     

    What if the doctor didn't sign or date the sports physical?

    The sports physical is an official medical record which MUST be signed and dated. Please take the form back to the doctor until they have fully completed it.

     

    FamilyID:

    What is FamilyID?

    FamilyID is a secure registration platform that provides you with an easy, user-friendly way to register for our athletic programs, and helps us to be more administratively efficient and environmentally responsible. Each season, you will use it to register the student athlete for the sport they're interested in, fill out the emergency contact information, health history, and permission forms, and attach a copy of their sports physical.

     

    How do I sign up for a FamilyID account?

    On or after June 1, the parent/guardian of the student athlete can register by clicking the REGISTER NOW button (top right on desktop, below on mobile) or by going to FamilyID.com and clicking 'Create Account'.

     

    1. Create your secure FamilyID account by entering the information of the account owner, the parent/guardian:
      • First and last names
      • E-mail address
      • Password.
    2. Select I Agree to the FamilyID Terms of Service. Click Create Account.
    3. You will receive an email with a link to activate your new account. (If you don’t see the email, check your e-mail filters (spam, junk, etc.).
    4. Click on the link in your activation e-mail, which will log you in to FamilyID.com

     

    What if I already have a FamilyID account?

    All you need to do is go to FamilyID.com and click "Login" in the upper right corner. (If you are having problems remembering your email or password, follow the instructions on the login page.)

     

    Does one family need a different account for each child?

    Nope! You can register multiple student athletes under one account. FamilyID will keep track of all their information separately.

     

    How do I register for a sport?

    Once you have signed in to FamilyID, you should see the Dashboard page. (If not, click "Dashboard".) It will say "I want to register for a program at:" followed by a search box; type "Upper Darby" (or "Drexel Hill"/"Beverly Hills" for middle school athletes) into the search box and click "Find". This should give you a link to register for the coming sports season.

     

    Once in the registration form, complete the information requested for each athlete. All fields with a red* are required to have an answer.

     

    **IMPORTANT NOTE: MAKE SURE YOU ATTACH A DIGITAL COPY OF THE SPORTS PHYSICAL IN THE 'PARTICIPANT INFORMATION' SECTION OF THE REGISTRATION**

     

    In order to submit the registration, you must click the green "Continue" or "Submit" button when your form is complete. There is also a "Save for Later" button, which will save your infomation but will not submit it to us.

     

    Do I have to fill out everything all over again each time I register?

    Nope! FamilyID will keep track of any recurring information you have entered for each student athlete. To auto-fill the saved information, begin to register the student athlete for their chosen sport. There should be a drop-down menu in the Name field in 'Participant Infomation'; select the name of your student athlete, and it will automatically fill in any available fields. Make sure all the information is correct and that their current sports physical is attached, then fill out any remaining fields and click the green "Continue" or "Submit" at the bottom of the page.

     

    What if I need to change or update infomation in the registration?

    You have full access to edit any personal information in FamilyID at any time. To edit the student athlete's registration, sign in to FamilyID and on the Dashboard page click "View or Update My Registrations". This will display all your registrations; select the one you wish to update and click "Edit".

     

    Is all this personal information secure?

    Absolutely. FamilyID is hosted on a cloud network administrated by Amazon and is protected by all of their online security measures. FamilyID was also thoroughly vetted by Upper Darby School District's legal department to make sure it followed all district, state, and federal privacy laws. FamilyID is fully HIPAA compliant.

     

    The only people who will have access to the student athletes' information are the person who created their account, the athletic trainer, the coach, and Athletic Department Administration.

  • Sports Physicals

    Each year PIAA regulations require that all student-athletes of the Upper Darby School District receive a physical examination prior to their participation in interscholastic athletics.  This physical must be performed and dated no earlier than June 1st and shall be effective, regardless of when performed during a school year, only until May 31st of that same school year.  For this upcoming school year, those valid dates are June 1, 2019 – May 31, 2020. Failure to turn in the physical and enter all required information online through FamilyID prior to the start of tryouts for your sport will result in a loss of participation for the upcoming season (until all necessary information is received, and at the discretion of the coach/athletic department).

    The sports physical is Section 6 of the PIAA Comprehensive Initial Pre-Participation Physical Evaluation (filled out and completed by the doctor). It must be uploaded to FamilyID, while students and guardians must fill out the rest of this information on the FamilyID website.
